Outputs: percentile and z-score for weight, stature and BMI-for-age and extended BMI percentile.
Formula, recorded as scope and not yet verified against a primary source: CDC 2000 growth charts for ages two to 20 years using the same LMS parameterization, plus the CDC extended BMI-for-age percentiles published in 2022 for BMI above the 95th percentile, which replaced the unstable extrapolation used previously.
