Formula, recorded as scope and not yet verified against a primary source: Sterling 2006: FIB-4 = age * AST / (platelet count in 10^9/L * sqrt(ALT)). Thresholds of 1.30 and 2.67 in adults under 65 rule out and rule in advanced fibrosis, with a raised lower threshold of 2.0 recommended above age 65.
