Formula, recorded as scope and not yet verified against a primary source: First-order one-compartment kinetics: elimination rate constant from two levels or from population estimates, then AUC24 = total daily dose / clearance, targeting an AUC24 over MIC of 400 to 600 mg*h/L per the 2020 ASHP, IDSA, PIDS and SIDP consensus guideline, which replaced trough-only monitoring.
